Change of Address

The following are policies in regards to changing your address in our records. These policies are for the protection of our shareholders.

Change of Address Policies:

  • Only a shareholder can change his or her address. Shee Atiká, Incorporated cannot accept address changes by friends or relatives of the shareholder.
  • Address changes can be accepted in writing or by telephone.  Written changes must include a valid signature. We do accept changes of address over the telephone after verification of pertinent shareholder information.
  • To change an address, Shee Atiká must have the shareholder's name as it appears on the shareholder records, his or her Social Security number and/or the shareholder's ID number. A signed and dated change of address form is preferred.
Shareholders can download the change of address form (in Adobe pdf format) from this website. Please print out the form, complete it and mail it to Shee Atiká.
